TELEPHONE NUMBER: 444-7599

LOCATION: Lafayette River Annex, 6500 Hampton Blvd., Norfolk, VA

PATIENT CONTACT REPRESENTATIVE: 444-7599

DESCRIPTION OF SERVICES: Provides a wide range of audiological services for all DOD activities within the Hampton Roads area. The primary objective is to prevent or reduce occupational hearing loss in personnel routinely exposed to hazardous noise operations. This is accomplished through training, audiometric monitoring, hearing protection fitting, certification of audiometric technicians, onsite assist visits, and professional diagnostic audiology assessment. A successful and effective Hearing Conservation Program increases the combat readiness of our military forces while decreasing the number and amount of compensation claims awarded. Since noise and its associated hearing loss have the capability to affect overall general health and quality of life, we as a product line, systematically maintain and direct our efforts at prevention and protection.

AVAILABILITY OF SERVICES: Open to all active duty and civil service personnel in the Hearing Conservation Program. The clinic is open Monday through Friday, 0730-1600.

APPOINTMENTS: Routine and active duty referrals should be entered in ERS. Priority or urgent referrals may be called in. Civil service referrals should be faxed to 444-3070. A copy of the DD2216 should accompany the consultation.

SPECIAL COMMENTS: The Occupational Audiology Product Line is in the Fleet and Family Medicine Directorate. Services provided by this clinic are in direct support of the Navy's combat readiness through management of the Hearing Conservation Program. For more information, contact the Occupational Audiology Product Line Leader at 444-7599.
